WELCOME TO THE DAMON SMITH WEBSITE.
Below you will find a description of what can be done using this website,
including a way to save a trip or two to my office.
We look forward to seeing you in our office and discussing your situation. In order for us to handle
your consultation as efficiently as possible, and to enable us to give you accurate and sound advice
as to your situation, we ask that you print and fill out the questionnaire listed in the button bar on the
left as completely and accurately as possible and bring it with you to the office interview. If you do
not have a printer or would prefer to fill in the questionnaire online and save an extra trip to the office
click the "Bankruptcy or Divorce Form Online "button to the left of this page

I promise the questionnaire will not be a waste of your time and it will probably save you at least
one trip to my office. The questionnaire only ask for the minimum information that we will need to
prepare the documents to be filed in your case. If a client comes to the office without the information
requested in the questionnaire I shall have no choice but to ask them to mail/fax or bring it back to
the office on a future date.
If it is inconvenient for you to come to an appointment at our office in Florence or Huntsville you
may avoid another trip by filling in the questionnaire online or by mailing or faxing the divorce or
bankruptcy questionnaire to our office. If you chose this option I will call you to discuss your case
within one day of receiving the questionnaire. In most cases you would then come to the office one
time to sign documents and go over any final details concerning your case with me. If choosing
this option and planning on filing a ch7 Bankruptcy or a Divorce case please include the Attorney
fee. Please see the fee schedule on the last page of the questionnaire for details.
