It is probably easiest to print the divorce questionnaire on this site and fill it in. If however you don't have a printer or don't
wish to use the online form, please feel free to bring the information below with you to my office .
I will need the home address and social security number for each of you. I'll also need to know how far each of you got in
school. If you have children younger than 19, I'll need (a) the full name, address, and social security of each child; (b) the
address and phone number where each parent lives and where each parent works; (c) the name of the employer for each
parent; (d) the income of each parent (including bonus, commission, overtime, and shift differential pay); (e) the cost of
work-related child care; and (f) the cost of health insurance, if any, for the children. You'll need to know the cost of health
insurance for family coverage minus the cost for the parent alone.
If you own a house together, and if one of you is going to sign it over to the other, you may want to bring a copy of your
deed. This will give me the information I need to create a quitclaim deed or statutory warranty deed so you can handle that
at the same time. The rest of the information I need to know you will probably bring with you in your head.
It is not necessary for you to resolve every issue concerning the divorce prior to coming to the office, however it will be
very helpful if you begin thinking about how you want to resolve the issues. The issues I would most like you to consider
are the following. Where will the children live. What do you wish done with the house. What do you wish done with the
other property, cars,stocks, bank accounts and debts. Will someone pay alimony and if so for how long, and how much.
Please also look at the document listed in the contested divorce section below. You may find it helpful when considering
the above statement. I believe it will help you focus on the issues and come to rational conclusions concerning resolving
those issues. In other words it will get you thinking in terms of what the court would be looking at if the case went to trial.
That will also help you when negotiating with your spouse.
WHAT WILL YOU NEED TO FILE MY CONTESTED DIVORCE CASE?
To represent you in a contested divorce case I will need the information listed in the uncontested section above. If
you choose to print the questionnaire or go to the online form and fill in the questionnaire, please skip the property sections
because they are covered in the long questionnaire below.
Within about two weeks after the case is filed I would like for you to return the questionnaire listed below
completed to the best of your ability. It will help me tremendousley in preparing your case for trial. It will assist me in
determining what motions need to be filed, what witnesses/documents need to be supoened, if an investigator needs to be
hired, if a deposition needs to be scheduled etc. etc. CONTESTED QUESTIONNAIRE The following language is
